Dear Kaipara Nurses An edition of the Korero was not published in March, due to the Forward Planning Workshop taking place. Speakers & change of meeting time for Nurse Project Meetings— The opportunity has arisen for informative speakers to present pertinent primary health issues at some of our meetings, the first being Tahi Morton this month, with a representative from Te Roopu Kimiora scheduled for the June meeting. Matters Arising from Feb 04 Minutes: · Tamariki Ora day funding – it appears that the funding may only be available to initiatives for the 0-5 age bracket. Julie to look into. · CBCP Portfolios – Reviewed and reported on by Julie – a copy of the report is available. · Cervical Smear Course – took place last week. Some Kaipara nurses in attendance. · Health Promotion Calendar - Circulated again. Rhonda from Te Ha will be presenting the initiatives that Te Ha have highlighted for promotion during the year. Te Ha welcome all services to join in with their planned activities. Business: Dept/Services Updates: Te Ha – Charlotte Lamb has resigned as at April 8th. Te Ha will be advertising for a new nurse. DARS / CARS – Looking for referrals on both services. Contact Margaret Hearn for criteria. Outpatients– Surgical Bus here on Monday (8th) for grommets. Majority of children from Whangarei. Feedback on the referral process required to ensure optimum usage by Kaipara residents. Stakeholder meeting still not taken place. Chris T to provide follow-up information. Diabetes Nurse- Diabetes Centre, Whangarei has commenced a 6 week Intro Course for newly diagnosed diabetics – held on a Tuesday from 9.30 – 11.30. Works on a rolling system, 6 weeks on, 2 off. If ++ interest from Kaipara residents, Health Shuttle could provide transport. Diabetes – Professional Development- Mereana, Diabetes Nurse, Manaia PHO is offering night classes to any nurse who wants PD on Diabetes. Julie/Mereana for details. Homeopath- Marion advised that she is in the process of organising a First Aid Homeopathic Course to be run over 5 weeks. Contact Marion for further details. Health Options: - Pam running a massage course over consecutive Sundays – to be confirmed. Windsong Colour therapy also being offered at the shop. Rhonda Zeilinski – Te Ha’s Health Promotion Plan for 2004:- (See Page 4 of Korero for details) Te Ha’s calendar is divided into ¼’s, then weeks, with each “promotion” rated: 1 = All Out – with Te Ha doing all the ground work, possibly holding an Expo and/or exposure at Woolworths. 2 = Te Ha do some of the hard work, put info up at the GP clinics, hold a quiz etc. 3 = Te Ha “turn up” – another dept/service has done the organising. Rhonda talked of the projects already highlighted on their annual planner. A new focus for the team this year will be the Garden Project – whanau will be encouraged to register, provided with free seeds and information on when, how, where etc to plant. Encouragement to produce their own organic food from a kaupapa maori perspective with healthier, more nutritional eating and some exercise thrown in for good measure! Te Ha have negotiated a price with the Dargaville & District News to advertise their promotions on a fortnightly basis. Forward Planning Workshop – Registration forms handed out. Discussion took place on attendance eligibility – it was decided that anyone who was a nurse, working in Kaipara for aservice delivering PHC or interacting with PHC services is eligible to attend. Regional vs Kaipara PHC Nursing Vision – to be discussed at the workshop. Speakers at Nurse Project Meetings – A number of speakers would like to present at these meetings. After discussion, it was decided that speakers could be invited to alternate meetings. To make sure business is completed in the allotted time, (whether there is a speaker or not) from now on, meeting would start at 12.00 (noon) so people could enjoy their lunch, with a 12.30 start for meeting proceedings. Tahi has asked to present the Disease Surveillance program he is leading which aims to raise awareness and create action to prevent gastro intestinal diseases and remove barriers to reporting gastrointestinal incidences. Introducing the Asthma Project Kia ora, my name is Cherry Waldron RCN and at present I am the Asthma Nurse for the Kaipara area and am responsible for the KCE Asthma Project. This project covers all persons with ashtma within the Kaipara region. My contract is held with KCI, however, I am sub-contracted to Te Ha o Te Oranga o Ngati Whatua and my office is based there. Ph (09) 439 1690—leave a message if I’m not available The Asthma project is a primary care initiative which is free to the public. The contract was formulated specifically for coverage of Asthma, but within these boundaries I am also aware of, and incorporate Pharmac Campaign Goals—these being:- To encourage persons with asthma, along with health professionals, of the importance of long term use of Inhaled Corticosteroids (ICS) versus reliever use as part of a total asthma management package. Promote evidence based management of Asthma. To encourage self management of asthma using the Self Management Plan. To increase the awareness of the health risks and costs of over-use of ICS. To encourage the regular review of asthma medication and titration to the lowest effective dose of ICS. I aim to work in conjunction with all the health providers in the Kaipara area to consolidate care for those in need of support, reviews/assessments, education (pathophysiology of asthma), advocacy, medication, enquiries and equipment. Focusing on ways of keeping asthma in control, observing triggers etc., and in consultation with the Doctors and Asthma Society, prepare management plans for acute episodes. REFERRALS The project relies on client referrals from all Kaipara health professionals, ie personnel from the Hospital, Medical Centre, Te Ha, Community Health and other community members eg teachers, individuals, family/whanau, friends, public events, self referral and currently through the “Breathe Easy” school programme. Consultations are mostly clinic, school or home based, booked to suit the client’s schedule and take approximately one hour per session per person. The schools have set afternoons at present and the education focuses on pathophysiology, self management and responsibility. Group Education is also available. Thank you for this opportunity to introduce my line of work and I hope that this information helps to trigger (pardon the pun) your thoughts of persons who may require support with their asthma. Cherry Julie's Column Greetings / Kia ora to all Primary Health Care Nurses in Kaipara. Welcome to autumn! The drive from Whangarei is beautiful and picturesque; we are lucky to live in such a beautiful country! OMAPERE WORKSHOP The workshop has been and gone and despite a few hiccups I think it went very well. The evaluations went away on holiday with Raewyn so I will get into them now that they have been returned to their rightful place! Generally, I have received favorable comments from a cross section of participants. With hindsight I would have done some things differently, (life’s like that), but generally it was a very good opportunity for those who attended. Out of all the suggestions and information presented, four key themes emerged: The development of an integrated PHC Nursing organization under the umbrella of the PHO which includes nurses from Te Ha, DMC, DN and PHN’s Development of an integrated, IT data network in Kaipara which enables members of the PHCT to communicate, liaise, plan and monitor pt cares more effectively Development of a PHCN pathway and Professional Development program which ensures nursing skills / competencies and knowledge are appropriate to the needs of the community. Development of Nurse led services/clinics which meets the needs of the community with a focus on Child, family, chronic Care management and obesity making use of such GP support strategies as standing orders and telephone triage. In essence, the nurses who participated in the workshop could clearly see the evolution of a more proactive, dynamic and integrated PHCN service. This will be an evolutionary process which incorporates consultation, partnership, and feet kept firmly on the ground. There is so much excellent work already been done by PHCN in Kaipara. Some of the above issues are outside the scope of my role or the Project Nurse Group; however that does not mean they can not be bought to the attention of Health Managers and those who are responsible for making decisions about the development of services in Kaipara. The other outcomes from the workshop include feedback from nurses about the sense of being valued as a nurse at this event, networking with colleagues and those from other organizations, learning about who is out there, what they do and how nurses can work more closely together. Some new opportunities presented in addition to the main program objectives as a result of nurses having some time to sit down and get to know nursing colleagues from other organizations better. Thank you to all of you who participated. At the Project Nurse Meeting I am hoping to get some discussion and agreement about where to from here. If any of the above issues are of particular interest to you and you would like to be involved in taking these forward, I would be very keen to hear from you. TRAINING IN ADOLESCENT HEALTH ASSESSMENTS Peter Watson, Specialist Adolescent Health Physician at the Centre for Youth Health in South Auckland, and Heidi McClintock, Adolescent Nurse Specialist has offered to come to Dargaville to provide training on the Adolescent Health Assessment tool, HEADSS. They recently ran a series of workshops in Whangarei for Adolescent Health workers and the feedback was excellent. Watch this space or contact me if you would like to be involved in the organization of this initiative. Follow-up care for children (0-16yrs) with long term healthcare needs.
