Matters Arising: Feedback on NIL positions: The three year Northland NIL pilots will end around July 06. When initiated, it was understood that if these positions were successful, they would be funded permanently by the DHB. However, the NDHB CEO is looking to base any decision on the pilot evaluation which will not be available for some months. Funding may therefore not be forthcoming. To show good faith and support for the positions, NPHO’s propose to negotiate allocating some excess Rural Workforce retention funding to cover 1/3 of the cost towards retaining these positions, of which there are currently 3. 1.0 FTEs in both Whangarei and Far North, with .8 FTE in Kaipara. New proposal would be for .5 FTE each to be allocated to Kaipara and the Mid North and the other two 1.0 FTEs retained. It was stated that better utilisation of the role could be made and discussion confirmed that the current position in Kaipara is valued with both KCI Board Members and Nurses not in favour of supporting the reduction to .5FTE. the KCI Board has instructed Chris Tipa to advocate for the best possible outcome for Kaipara. Inservice Education: Julie spoke on some recent inservices held and advised that Chris Perkins, a Gerontoligist specialising in dementia and with a particular interest in Rural areas, will is scheduled to provide the next inservice. Contact Julie for more details. Business: Terms of Reference: A copy of the Nurse Management Group TOR (June 2000) distributed with the Korero. The TOR were discussed at Nurse Project Meetings on Oct & Nov 02, and in Feb 03 where it was stated that they still looked relevant. In July 03, it is minuted that the task of reviewing the TOR would be given to the new NIL once she had been become familiar with the project and situation in Kaipara. However, no mention of the TOR has been minuted since and perhaps was never identified as a task to Julie. The history of the Nurse Management/Leadership group vs the open forum today was discussed. Any comments or suggestions on the TOR to be sent to Julie by 15th March and they will be presented at the next meeting. Presentations: It was noted that any presentation should include how other nursing groups can assist the presenter’s dept or role. Also for consideration, some allied provider presentations might benefit this group. Presentation by Celeste Sherman, Out Patients Dept, KAHS: Celeste shared her nursing history and background with the group Her role in OPD involves much phone and paperwork, but nursing skills are also called upon. She manages the many and varied outpatient clinics that are held by visiting specialists. They include: Surgical, Medical, Diabetic, Geriatric, Gynaecological, Orthopaedic, Retinal Screening, Paediatric, ENT, Surgical Bus. Clinics are held at various frequencies from twice a month to twice a year, and are often moved (dates), reduced (in frequency) or cancelled all together, like the pacemaker clinic. It was noted that NH’s agreement with KCHT states that services to the Kaipara would not diminish when the hospital was downgraded. Zoe Tipa, KCI rep on the KCHT Board, is willing to take a written report to the KCHT Board on any matter regarding the reduction of services. The department is currently under instructions from the NH Out Patient Project Manager to book appointments for all patients who have been waiting longer than 12 months. The directive is from the MOH and is in line with a Labour party election promise to reduce time on waiting lists. This can be difficult to manage, as new referrals may include more urgent cases. Celeste stressed that any referral to a specialist should include as much information as possible, including social situations. DNA’s – if a client does not attend, many specialists will cut them from the list and not see them again till receipt of another referring letter from the GP. The recent change from sorting local mail in Dargaville, to having it sorted in Auckland, has seen an increase in the no. of clients not receiving their appointments due to wrong addresses. Other problems include clients not having transport or phones. Other nursing groups can assist OPD in reducing DNA’s by ensuring that any client on a waiting list has provided up to date contact details i.e. address, ph, alternative contact person etc. Nurses could also advise if the client has had the required procedure done privately. This will allow other pts to be included in, or moved up, the list. Other: New NH DON, Denise Brewster-Webb – to invite her to the May 06 meeting. Venue – Everyone was happy to change usual meeting venue to the Nurse’s Lounge from now on. ALL NURSES ARE WELCOME Julie's Column Kia ora PRESENTATION At the April project nurse group meeting we will be having a presentation delivered by Johny Wilkinson, Manager of Northland Disability Empowerment and Advocacy Services. He will be introducing a new resource that has been developed for people with disabilities in Northland to ensure people are able to access support and disability services more easily. TERMS OF REFERENCE In addition we will be addressing the adoption of the groups Terms of reference (TOR). At the last meeting we looked at the ‘original’ groups TOR from 2001, the Te Tai Tokerau Regional Nurse Leadership Groups TOR, and a draft that I had also written. It was agreed that people would review these for their suitability for our group’s purposes, and if they had any comments, submit them to me by the 15th. I received some feedback and have incorporated the suggestions into the attached document. KAIPARA CARE INC PHO RURAL RETENTION FUNDING The PHO has received annual funding from the MoH for recruitment and retention of primary health care (PHC) staff. The KCI Board has decided the funding be allocated twice yearly based on submissions from PHC staff in Kaipara. Nurses in Kaipara may like to consider applying for this funding. Application forms can be obtained from KCI. The project Nurse Group could apply for funding for a project that would promote recruitment and retention of Kaipara Nurses in Primary Care services. This will be discussed at the next meeting. Please bring any ideas along. SMOKING CESSATION TRAINING Northland Health Smoke free coordinators have negotiated with the Heart Foundation QUIT trainers to provide a training course for Nurses in Northland. If there is enough interest, the training could be delivered here in Kaipara. There are a limited number of places 9 – 20 per course. Please contact me if you would be interested in attending this free course. Attendance at the course would mean you will be able to provide effective smoking cessation support/ set up smoking cessation ‘clinics’, issue QUIT cards sp that people can access Nicotine Replacement patches for $5.00 from the chemist, and link into a smoking cessation network. FAREWELL TO JUNE HENWOOD, TEAM LEADER AT TE HA O TE ORANGA O NGATI WHATUA All good things must come to an end and so June thinks its time for her to hang up her Te Ha hat. June has been an awesome advocate and Nursing leader for Te Ha over the last 2 years or so and she has led her team to provide the best possible nursing and Health Promotion care they can, whilst at the same time, supporting, leading and working with Nurses and health care providers across Kaipara. Her generosity of time, energy and Aroha for both nursing and health colleagues and her “patients” has been commendable. We will miss you June and your tremendous spirit, we wish you all the very best with Gary in your new retirement. Have fun and remember to visit us regularly. Julie Palmer Nursing Integration Leader Kaipara Care Inc (PHO) Ph: 09 439 7149 X 6885 Mobile: 027 439 711 Draft Terms of Reference DRAFT TERMS OF REFERENCE For Kaipara Project Nurse Group Vision/Purposes To provide a forum for the support of integrated nursing services within Kaipara, to promote the health and wellness of the community. Functions of the Stakeholders 1. To commit to a shared vision and facilitate processes that make this vision become a reality. 2. To provide a cohesive forum for networking, sharing issues and new initiatives including feedback from courses and conferences. 3. To enhance integration and collaboration of nursing and health services in Kaipara by identifying, completing and evaluating at least 1 project per year. 4. To make recommendations to appropriate groups on issues that effect the Kaipara. Membership/Representation of Stakeholders All nurses whose work encompasses the Kaipara. It is acknowledged that there is a wide variety of knowledge, skills, expertise and roles amongst nurses in the Kaipara and these are each respected and valued by the Project Nurse group as a whole. Responsibilities Commitment to attend monthly meetings, or send a nursing representative. To communicate and discuss issues with those areas they represent and feed back their input To network with areas/speciality they represent. Minutes and draft agenda will be circulated via email two weeks before the meeting. Any specific agenda items to be submitted to the NIL by one week prior to the meeting (held on the first Wednesday of each month) Korero and final agenda to be circulated on the Thursday prior to meeting Frequency of meetings Meetings are held monthly on the first Wednesday of each month excepting January. The venue for the meetings is the Nurses Lounge in the Community Health Building. March 2006 Letter of Introduction Letter of Introduction: Kia ora tatou, I’d like to begin by introducing myself. My name is Amanda Bradley and I am part of the Smokefree Team within the Public Health Unit at Northland DHB. My role in the team has two key parts. Firstly, I support Pauline Allen-Downs to provide a smoking cessation service at Whangarei Hospital to patients and staff. Secondly, I am implementing a project called Systems First, which is guiding the development of smokefree hospital systems. Part of systems development is to examine how well resourced we are, as a community, to support people to make changes to their smoking. Being well resourced means making smoking a priority for reducing inequalities in health. It means that staff have the time and the training as well as information in a variety of forms to offer patients who want to make changes to their smoking. The Smokefree Team are committed to working with our communities to make best use of our resources. Over the next twelve months we aim to support the training of practice nurses to become “Quit Coaches” and to offer patients subsidised Nicotine Replacement Therapy (two days free training through the Heart Foundation). We will also be offering free training to front line nursing staff in Effective Brief Intervention. Presentation: By Jonny Wilkinson, Manager for Te Tai Tokerau Disability Empowerment Advice Support: DEAS’s contract with the MOH has changed from one of advocacy for people with disabilities, to advising other organisations who offer services and advocacy to clients with disabilities i.e. NorthAble, Alzheimers Socieity Northland Inc, The Brain Injury Association, Arthritis NZ, CCS Northland Inc, Deaf Assoc, Epilepsy Northland, Northland Multiple Sclerosis Society, Royal NZ Foundation of the Blind, Northland Down Syndrome, Northland Stroke Foundation, Parent to Parent Northland, Parkinsons Northland. Pads with this contact information were distributed. Business: Report by Cherry Waldron: The importance of the NIL position in TTT and Kaipara was discussed. Cherry had drafted a letter to be sent to the NDHB in support of retaining the positions, and Kaipara’s as a .8FTE in particular. The letter was circulated and feedback was invited. The group agreed to send the letter to the DHB which Cherry will do in the next couple of weeks. Julie advised that this was time for the group to get involved in a new job description for the position. Her contract ends in July, and she will not be seeking to extend. Cherry advised that she now works part time for both District Nursing and Te Ha. She will be taking 3 months leave from 6th June to the end of August. This will leave the nurse position vacant on the KCI Board. The Board’s policy is not to accept proxy representation unless in special circumstances. Cherry would like to remain on the Board and stand again next year. An alternative would be for Cherry to stand down and for the group to nominate another representative. Cherry will bring the issue up with the KCI Board at the April meeting. All KCI Board members are nominated annually, as per their constitution. Any changes would need to be passed at an AGM. Rural Workforce Retention Funding – submissions close 19th April. It was proposed that this group may like to make a submission of an annual amount to cover study leave and other appropriate items. A subcommittee from this group could be formed to look at submissions. M/S June/Helen that the Nurse Project Group makes a submission for the funding. Carried. SIA funding has been extended to Kauri Coast Resthome and Hospital to cover 20 bed nights for members of the community who are not sick enough for hospital but require care. Strict criteria must be met. Contact Cherry or KCI for details. Presentation to June Henwood After many years of nursing and leadership in Northlland and in the Kaipara, June “retires” on April 21st. ALL NURSES ARE WELCOME Julie's Column Kia ora RURAL RETENTION FUNDING Following on from our last meeting, an application has been sent to the KCI PHO Rural retention sub committee for funds to be made available for the Kaipara Project Nurse Group (KPNG) to support professional development t of Kaipara Nurses. The application was for $10, 000.00 and if funding is approved, it will be available to all Nurses who work in Kaipara. This is important because it creates opportunities for Nurses to access resources which will enhance knowledge, skills, and attitudes, improve health care and in turn health outcomes. NURSING INTEGRATION LEADER (NIL) POSITION The NDHB has approved funding for the NIL position for the next 3 years at a .5 level. Currently the position is funded for .8. Over the next few months nurses will be able to have a say in how they would like to see the role develop. The intention of this position was to improve health outcomes by working on specific health issues, with Nurses, other health providers and the community or specific population groups The range of work done around developing and growing Primary Health Care services in Kaipara has been substantial, having said that there are still many areas for Nurses to contribute to improving health services and outcomes for virtually every population group across the lifespan. If you are interested in this kind of work you might want to think about putting yourself forward either to be involved in the establishment of the new position or in applying for the position yourself. CHRIS PERKINS IN KAIPARA 14TH AND 15TH JUNE Dr Chris Perkins is author of the NZ Dementia Guide. She has agreed to come to Kaipara and do 2 presentations on managing Alzheimers and Dementia. These sessions will be open to all nurses and health providers that have an interest in or work with patients who suffer from Dementia/ and or Alzheimers. The dates are June 14th and 15th and a panui will be sent out shortly. KAIPARA NURSES FORUM Jen Udy has suggested we consider having a Rural Nurses Forum here in Kaipara. This would be an opportunity for shared learning, working together giving and receiving some inspiration. If any of you are interested in this idea, and have some suggestions about what you would like to see, let me know, we can discuss this further at the Project Nurse meeting next week.
