MINUTES OF THE NURSE PROJECT MEETING HELD ON WEDNESDAY, 3 AUGUST 2005
AT 12.15, CONFERENCE ROOM, COMMUNITY HEALTH, DARGAVILLE HOSPITAL

Business:
Kaipara Leadership Development Programme
Cherry began with a quote in Maori to celebrate Maori Language Week. She followed this up with two more quotes: “Become the change you wish to see” – Gandhi, and “Doing good id good for you” – Mother Theresa.

Wendy was introduced in her role for Kaipara District Council, and spoke to the Kaipara Leadership Development Programme which is being run by the Auckland Leadership Institute (part of Auckland University) (See KDC website). This programme is Kaipara specific, it will run for 5 hours per week, 18 days over an 18 month period, covering projects which will continue after the programme is finished. There are 15 places and participation is free. There is no age limit and anyone in the Kaipara community can attend. The application form will be on line in a few weeks for registration of interest. Wendy asked all to think of community members who would qualify to participate and to urge them to join the programme.

Reminder on Educational Sessions - Ward 2
Jen reminded everyone of the Elder Abuse sessions to be held on Ward 2 tomorrow. These are being run by Age Concern and are 1 hour long. There are several sessions throughout the day.
Also Pain Management sessions between 2 pm and 4 pm today being run by DMC.

NIL Role
The Nursing Researchers considered that as there was 1 year left to run on Julie’s role as NIL, and there had been a number of changes in nursing staff on site, it would be a good idea for the nurses to know what her role is. Julie is setting up a meeting with Judy Harris and DMC.

Regional Perspective
Five Nurse Practitioner positions have been approved which is a first in New Zealand. These are initially to Iwi Providers and have ongoing funding. Ministry of Health Nursing Scholarships have provided funding to pay for nurse practitioners. There should be 12 practicing in Northland by the end of next year. The five positions are in Primary Health Care and are funded for specific scope of practice and where needs are. This is determined through the DHB, MAPO and practitioners. These are unique positions, still in their infancy.

A visit from America of the CEO of the Nursing Consortium who was invited and sponsored by the Ministry of Health, Francis Hughes and an Eisenhower Scholarship gave insight and shared ideas on nurse led health centres. We were also able to provide and insight into what we do.

A steering group comprising the Registered Nurses Leadership Group and those completing their Masters next year has been set up to look at the possibility of setting up a nurse led health centre in Northland. The first meeting of this group is scheduled for next month.

Prescribing is still being sorted out. It is desirable, but there are some who do not have this authority. The aim is for Nurse Practitioners to have prescribing rights within their scope of practice.

Pay Parity
Voting forms were received too late to vote. It is hoped an extension will be granted. Zoe investigating. It is vital that all votes to be heard.

Orientation
Sent to DMC for new nurses. Julie waiting to hear back. Not gone to Jen yet. Recruitment and Retention team has not met yet. Pay parity is one of the challenges for recruitment. This was discussed a couple of meetings ago at the Project Nurse meeting. Minutes and further information from June.
It was agreed that shared orientation had not been worked through thoroughly by DMC, the ward and Te Ha. Julie needs time with Nurse Leaders to discuss this.

Return to Nursing
NDHB will be running a “Return to Nursing” programme in the new year. Anne McKillop is working with the polytech to set this up. The programme needs to be run at the end of the academic year because of the capacity of the ward. The programme is very expensive for students, it is uncertain at this stage whether NDHB will be providing funding.

Through the Nursing Council Judy Harris has managed to arrange for one nurse to update via DMC and Ward 2 due to lack of provision for training in the Region. A suggestion that DMC students could be encouraged to enter the nursing profession was discussed, but due to difficulties involving patient privacy it was agreed that a tour of services might be the only option.
Nurse Rep Report
Pharmacy fund available for patients who cannot pay for scripts. Pharmacist can buy in to system as well. Regarded as appropriate for Pharmacist to access fund. Funding has been carried through.

PHO looking toward performance management programme which will reward PHOs for performance and improvements to community health over a specified range of health problems, ie there is financial incentive to perform.

MeNZB got around 86-87% - good response. Outreach also progressing and community responding well despite negative media coverage.

SPRING IS DEFINITELY HERE, and doesn’t it feel great! I know that you have all worked very hard this winter, I hope you are able to get out there and enjoy the warmer weather, doing whatever it is that makes you feel great to be alive!

PRIMARY HEALTH CARE NURSING(PHCN)
The development of the PHCN workforce continues to evolve in a dynamic and systematic manner. In Te Tai Tokerau, a Nurse Practitioner (NP) is now employed within the PHC setting at Te Hauora O Te Hiku O Te Ika Trust. Twelve Nurses are currently completing their clinical Masters and will be able to apply for NP status by the end of 2006. Over 20 PHCN in Northland are studying at Post Graduate level.

The Institute of Rural Health in collaboration with the Clinical Training Agency (CTA) has funding available for the Post Graduate Diploma in Health Sciences (Advanced Nursing) Primary Health Care (I have information available for anyone who is considering rural PHC study).

In Te Tai Tokerau, the NDHB and its partner the MAPO have announced the allocation of funding for 5 Nurse Practitioner positions. A group of Iwi Provider Nurse Leaders have started working together to ensure the positions are successfully established and that the resources available are fully maximized. In Kaipara, Rhonda Zielinski has taken on the position to manage the establishment phase at Te Ha. Rhonda will be giving a presentation at the October Nurse Project meeting on this significant new addition to our workforce. Welcome back Rhonda!!!

VISIT TO KAIPARA BY THE ACTING CHIEF NURSING ADVISOR TO THE MINISTRY OF HEALTH
Pauline Cook is the Acting Chief Nursing Advisor at the Ministry of Health, she took over from Francis Hughes and will be there until the new Nursing Advisor, Mark Jones, starts later this year. Pauline is coming to learn about the range of nursing initiatives going on in Northland. We will be hosting her on Friday morning, 16th September. A Powhiri will be organized by the Te Ha Nurses, and all nurses will be welcome. Further planning is taking place this week, contact me if you have any suggestions that would help make her trip here informative and memorable!

PAY PARITY FOR PRIMARY HEALTH CARE NURSES (PHCN)
Last week I attended the NZNO PHC seminar on pay parity for Primary Health and aged care. It was very well run, interesting and informative. Approximately 15 Nurses from around Northland attended. NZNO is seeking pay parity for PHCN with their DHB colleagues. The importance of Pay parity for PHCN and aged care arises because the inequity in pay and conditions means there is the potential for a massive PHCN exodus to hospital /DHB positions as PHCN fall behind financially, from their DHB colleagues. This will have a direct impact on:
Recruitment and retention for Nursing and in particular PHCN.
Maintaining and valuing the wealth of experience and knowledge of PHCN
Achieving the goals of the PHC Strategy.

The challenges with this campaign are considerable, however the benefits to be gained by all nurses, and the communities they work in, is also significant. One of the key issues I gleaned from the seminar was the fear that a pay increase will have to be covered by increased patient fees to the GP. GPs currently are employers, of PHC Nurses, and the Government is the funder. Pressure on the Government to allow the DHB Nurses pay increase was negotiated. It is timely now for the DHB nurses to lend their support to their PHC colleagues. A comprehensive campaign is being waged by the NZNO. Your support will be appreciated and needed.


HEALTH PROMOTION (HP)
PHO’s are allocated funding to deliver, coordinate or support HP with their registered population. Because of the size of our PHO, (Approximately 12,500 registered with KCI PHO) the amount is around $20,000. The process for allocation has been negotiated through the KCI Board, the KCI HP subcommittee and the Kaipara HP Network. We have just completed the first round and 6 Projects have been funded. See the Aug 05 mins for details.

In November, KCI will again be allocating funds for HP programs. If you would like to know more about developing a Health Promotion project/program, don’t hesitate to contact me, I welcome your enquires. It is intended that HP funding will be allocated biannually, in May and November.


INFLUENZA PANDEMIC PLANNING FOR KAIPARA / NORTHLAND
A presentation will be given by Dr Jonathan Jarman and the NDHB Public Health team on the Influenza Pandemic which is expected to reach NZ, and be a serious threat to the health of our communities, particularly those who already experience disparities in health, i.e. the elderly, those with chronic diseases, and children.

We, as health providers and promoters, will need to consider how we would cope should a severe influenza pandemic strike Northland.
