Projects – Archive Information

The Co-ordination Contract that initiated the Well Elderly, Diabetes, Asthma, Immunisation and Nurse projects has now ceased.

Diabetes, Asthma and Nurse are now covered under separate contracts i.e. Free Annual Diabetes Check, Asthma Nurse Services, and 3 years of Nursing Innovations Fund’s for a Nursing Integration Leader. The Immunisation project has evolved into an Outreach Programme covered by SIA (Services to Improve Access) Funding and the Well Elderly programme is covered by NDHB funding a .2FTE Well Elderly Monitoring Nurse position.
